The acuteness of the influenza problem and fundamental importance ofresearch aimed at uncovering its viruses places the theme of virai involvement in vaccines among the most interesting and urgent problems in the field of medicai science today.

Influenza is a major respiratory pathogen causing annual outbreaks and occasional pandemics. Influenza vaccination is the major method of prophylaxis. Currently annual influenza vaccination is recommended for groups at high risk of complications from influenza infection such as pregnant women, young children, people with underlying disease and the elderly, along with occupational groups such as healthcare workers and farm workers. There are two main types of vaccines available: the parenteral inactivated influenza vaccine and the intranasal live attenuated influenza vaccine. Influenza continues to remain among the most important respiratory infections causing annual seasonal epidemics and the occasional pandemic. The 2009 H1N1 flu virus (referred to as “swine flu “early on) is a new influenza virus strain that is causing illness in people. This new virus strain was first detected in people in the US in April 2009 and is spreading from person-to-person worldwide, probably in much the same way that regular seasonal influenza viruses spread. On June 11, 2009, the World Health Organization (WHO) declared that a pandemic of 2009 H1N1 flu was underway. On September 15, 2009, FDA approved four Influenza A (H1N1) 2009 Monovalent Vaccines for the active immunization of individuals against influenza disease caused by pandemic (H1N1) 2009 virus.

Track 1: Status of vaccines research and methodology

The word “vaccine” originates from the Latin Variolae vaccine (cowpox), which Edward Jenner demonstrated in 1798 could prevent smallpox in humans.  Today the term ‘vaccine’ applies to all biological preparations,  produced from living organisms,  that enhance immunity against disease and either prevent (prophylactic vaccines) or,  in some cases,  treat disease (therapeutic vaccines).  Vaccines research and development are administered in liquid form, either by injection, by oral,  or by intranasal routes. Vaccination is the administration of antigenic material (a vaccine) to stimulate an individual's immune system to develop adaptive immunity to a pathogen.  Vaccination is the administration of antigenic material to stimulate an individual's immune system to develop adaptive immunotherapeutic to pathogen.  A vaccine is a biological preparation consisting of an agent that closely resembles a disease-causing microorganism and is made from weakened or killed forms of the microbe, its toxins or one of its surface proteins.

Track 2:  Influenza virus - Immunology  and Genetic Features

Immunology is the branch of biomedical sciences concerned with all aspects of the immunological research in all multicellular organisms.  Antiviral resistance in influenza may not only develop during treatment but also sometimes transmit widely to replace susceptible strains in the absence of drug pressure.  Although instances of antiviral drug resistance among influenza viruses have occurred during antiviral therapy, resistant influenza strains have spread widely in the absence of such drug pressure.  Influenza occurs in outbreaks and epidemics worldwide, mainly during the winter season. Although acutely debilitating, influenza is usually a self-limited infection; however, it is associated with increased morbidity and mortality in certain high-risk populations.

Track 3: Transmission and Diagnosis of H1N1 Influenza Virus

H1N1 flu is also known as swine flu.  It's called swine flu because in the past, the people who caught it had direct contact with pigs.  That changed several years ago, when a new virus emerged that spread among people who hadn't been near pigs.  In 2009, H1N1 was spreading fast around the world, so the World Health Organization called it a pandemic. Since then, people have continued to get sick from swine flu, but not as many.  H1n1 diagnosis is recommended on suspect cases of novel influenza A H1N1 (swine flu).  Active infection with swine flu diagnosis influenza can only be confirmed by polymerase chain reaction (PCR) assays, including sub-typing and further characterization by other special PCR assays.

Track 4: Influenza Epidemiology and Disease Management

Influenza occurs in distinct outbreaks of varying extent every year.  The influenza epidemiology pattern reflects the changing nature of the antigenic properties of influenza viruses, and their subsequent spread depends upon multiple factors, including transmissibility of the virus and the susceptibility of the population.  Influenza A viruses, in particular, have a remarkable ability to undergo periodic changes in the antigenic characteristics of their envelope glycoproteins, the hemagglutinin and the neuraminidase.  Influenza is a highly infectious disease which can occur as a pandemic, epidemic, outbreak and in form of sporadic cases.  While in home isolation, patients and other household members should be given infection control instructions, including frequent hand washing with soap and water.  Use alcohol-based hand gels (containing at least 60% alcohol) when soap and water are not available and hands are not visibly dirty.  Patients with H1N1 influenza should wear a face mask when within 6 feet of others at home.

Track 5: Influenza Treatment & Therapies

Influenza treatment and therapies are largely supportive and consists of bed rest, increased fluid consumption, cough suppressants, and antipyretics and analgesics.  Patients should be encouraged to stay home if they become ill, to avoid close contact with people who are sick, to wash their hands often, and to avoid touching their eyes, nose, and mouth.  Patients who develop flu like illness (i.  e,  fever with either cough or sore throat) should be strongly encouraged to self-isolate in their home for 7 days after the onset of illness or at least 24 hours after symptoms have resolved,  whichever is longer.  To seek medical care, patients should contact their health care providers to report illness (by telephone or other remote means) before seeking care at a clinic, physician's office, or hospital.

Track 6: H1N1 Influenza Vaccine

H1N1 influenza is sometimes called "swine flu" because it is usually found in pigs.  You cannot become infected with H1N1 influenza from eating pork products.  H1N1 influenza virus can spread from one person to another through small droplets of saliva that are expelled into the air when an infected person coughs or sneezes.  The virus can also be passed through contact with objects the infected person has touched, such as a door handle or other surfaces.  Becoming infected with influenza is much more dangerous to your health than receiving the vaccine to protect against it.  Most people with H1N1 influenza have recovered, but the virus has caused some deaths.  Like any medicine, this vaccine can cause side effects, but the risk of serious side effects is extremely low.

Track 7: Recent Advancement in Influenza Vaccine

Influenza can induce serious acute respiratory distress syndrome (ARDS) and non-conventional treatment like extracorporeal membrane oxygenation (ECMO), can be safely instituted in highly specialized centres.  Due to a rapid progression of respiratory failure, transfer of these patients to a referral centre on mechanical ventilation is often infeasible.  Therefore the start of ECMO flu treatment and care at the peripheral hospitals and transportation on ECMO could be a viable option.  The creation of an ECMO team, with experts in the treatment of ARDS, seems a key aspect for the safe management of these patients, including when the most advanced technique are not immediately available.

Healthy and at risk children are susceptible to the morbidity and mortality associated with viral-induced respiratory diseases, including respiratory syncytial virus (RSV) and influenza. Track 11: Respiratory tract infections

Respiratory Tract Infections (RTIs) can spread in several ways. If you have an infection such as a cold, tiny droplets of fluid containing the cold virus are launched into the air whenever you sneeze or cough. If these are breathed in by someone else, they may also become infected. Infections can also be spread through indirect contact. For example, if you have a cold and you touch your nose or eyes before touching an object or surface, the virus may be passed to someone else when they touch that object or surface.
